2. Authorization

I authorize Force 1 Solutions ("the Company"), and any consumer reporting agency or other third party it designates, to conduct a background investigation in connection with my application for employment or continued engagement with the Company. I understand the investigation may include verification of the information I have provided and the checks I have initialed below.

I understand that a photocopy or electronic copy of this authorization is as valid as the original, and that this authorization remains valid for the duration of my application, employment, or engagement, unless I revoke it in writing, to the extent permitted by law.

4. Disclosures

Fair Credit Reporting Act (FCRA): A consumer report or investigative consumer report may be obtained about me. I have received a copy of the "Summary of Your Rights Under the Fair Credit Reporting Act" and understand I may request a copy of any report and dispute inaccurate information.
Criminal history: A conviction record will not automatically disqualify me from employment. The Company will consider the nature of the offense, how much time has passed, and its relevance to the position, and will comply with applicable federal, state, and local "ban-the-box" and fair-chance laws.
Driving history: If the position involves operating a company vehicle, I understand that a valid driver's license and an acceptable driving record may be a condition of employment and ongoing eligibility.
